Veteran health check

What is a Veteran Health Check?

A Veteran Health Check is a comprehensive assessment aimed at evaluating various aspects of a veteran's health, including:

Medical history

Physical examinations

Mental health evaluations

Social well-being

The goal is to identify any health concerns early and provide appropriate interventions or referrals to enhance overall health and quality of life.

Eligibility

All those who have served in the ADF can access a comprehensive One-Off Veteran Health Check, any time after transition.

A Medicare rebate is available for this.

You do not need to be a DVA client to access this health check.

The Annual Veteran Health Check is available for those who transitioned after 30 June 2019. DVA fully-funds an Annual Veteran Health Check every year for the first five years after you transition. You will need your DVA Veteran card to access this.

What to Expect During the Health Check

During your visit, our experienced General Practitioners (GPs) will:

  1. Collect Information: Gather detailed medical history, including any service-related health issues.

  2. Conduct Examinations: Perform necessary physical and mental health assessments.

  3. Develop a Management Plan: Create a personalized health plan addressing identified concerns.

  4. Provide Referrals: Recommend appropriate interventions or referrals to specialists if needed.

  5. Where clinically required, GP can:

  • provide necessary prescriptions and referrals

  • conduct regular reviews of your medication and treatment

  • coordinate all your health services

  • prepare a health care plan, health assessment or case conference for you under certain conditions

  • in some cases, perform acupuncture.

   6. Where needed,  GP may refer you for:

  • Allied Health treatment such as physiotherapy, diabetes education or podiatry

  • medical specialist treatment such as cardiology or hearing services

Coordinated Veterans' Care (CVC) Program

The practice participates in the Department of Veterans' Affairs (DVA) Coordinated Veterans' Care (CVC) Program, which is designed to provide proactive, patient-centered healthcare to veterans with chronic conditions.

 

The program aims to reduce the risk of unplanned hospital admissions by offering coordinated care tailored to the individual needs of each veteran.

The CVC Program is available to:

  • Gold Card holders with one or more chronic conditions.

  • White Card holders with an accepted mental health condition while in service.
